After discharge, the most useful family plan is usually simple: names, tasks, dates, and backup coverage. Avoid assigning work to "someone."
Start with the first week
- Medication pickup and pill setup
- Follow-up appointment scheduling
- Transportation to appointments
- Meals, groceries, hydration, and basic household needs
- Home safety setup and equipment delivery
- Insurance, paperwork, portals, and bills if needed
- Family updates and daily check-ins
