Why Family Elder Planning Exists

Family Elder Planning was created for adult children who suddenly find themselves responsible for an aging parent after a hospital stay, fall, surgery, illness, or health decline.

Hospital discharge can move quickly. Families are often handed instructions, medication changes, follow-up tasks, and safety concerns all at once. It can be hard to know what to ask, what to track, and who in the family is handling what.

This site is designed to help families get organized during the first 14 days after discharge. The checklists, care logs, and planning prompts are meant to help you prepare better questions for the hospital team, doctor, pharmacist, home-health agency, or other licensed professionals.
